Chronic Care Management Services (Medicare patients)

Ridgeview partners with ChartSpan to provide care management services. Care management is a Medicare program that gives you access to care in between your regular office visits. The goal of the program is to keep you healthy, offer your assistance, and prevent any conditions you have from worsening.

How does it work?

ChartSpan is a remote extension of your Ridgeview team. This program uses nurses and clinicians to help you manage your health needs outside of Ridgeview office visits. These team members then report all updates back to your Ridgeview provider to keep them informed.

Once enrolled, your team members will assist you in your personalized health journey each month — offering help with everything from answering questions to assisting with medication refills and appointment scheduling.

How much does it cost?
