Patient Financial Responsibility
Thank you for choosing Ridgeview as your care provider. We are committed to providing you with quality and affordable health care. To better address your questions regarding patient and insurance responsibility for services rendered, please review our payment policies. Insurance
Ridgeview participates in most insurance plans, including Medicare. If you are insured by a plan Ridgeview does business with but don’t have an up-to-date insurance card, payment in full for each visit is required until your coverage can be verified. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage. If you are not insured by a plan Ridgeview does business with and choose to use our services, payment is expected at each visit. If you are not insured by any carrier, you are encouraged to apply for insurance coverage through MNSure.org. If you have exhausted all insurance coverage options without success, you may qualify for Ridgeview Community Care. Ask for an application by calling 952-442-8054.
Copayments and Deductibles
All copayments and deductibles must be paid at the time of services. This arrangement is part of your contract with your insurance company. Failure on the part of Ridgeview to collect copayments and deductibles from patients can be considered fraud. Ridgeview may request prepayment of anticipated deductibles, coinsurance and copays for upcoming services.
Prepayment Requirements
Prepayment of anticipated charges may be required in cases of no insurance or for certain services such as cosmetic surgery and non-covered procedures. Please be aware that some—and perhaps all—services you receive may be non-covered or not considered reasonable or necessary by Medicare or other insurers. You must pay for these services in full.
Self-Payment Requirements
An uninsured discount will be applied to all self-pay accounts at the current discount rate for applicants that meet the qualifications. Cosmetic and self-referred services are excluded from this discount.
If you are unable to make full payment of the account balance, a reasonable monthly payment arrangement may be made between the responsible party and Ridgeview.
Proof of Insurance
Ridgeview must obtain a copy of your photo ID and current valid insurance to provide proof of insurance. If you do not provide us with correct insurance information in a timely manner, you may be responsible for the balance of a claim.
Claims Submission
Ridgeview will submit your claims and assist you in any way we reasonably can to help get your claims paid. Your insurance company may need you to supply certain information directly. It is your responsibility to comply with their request. Please be aware that the balance of your claim is your responsibility whether or not your insurance company pays your claim. Your insurance benefit is a contract between you and your insurance company; Ridgeview does not participate in that contract.
Coverage Changes
If your insurance changes, please notify Ridgeview before your next visit so we can make the appropriate changes to help you receive your maximum benefits. If your insurance company does not pay your claim within a reasonable time, the balance will be billed to you.
Nonpayment
If your account is over 90 days past due, you will receive a letter from Ridgeview. If a balance remains unpaid, Ridgeview may refer your account to a collection agency and you will need to pay in full or make reasonable payment arrangements prior to scheduling your next visit and our physicians will only be able to treat you on an emergency basis until arrangements have been made.
