Nursing Centers/Assisted-Living Residences
Social workers offer inpatient education and guidance in the selection of a nursing home or assisted-living facility to meet your health care needs after your hospital stay.
Nursing centers are facilities licensed to provide 24-hour, skilled nursing services under the supervision of a medical director. The center’s caring team consists of nurses, physical, occupational and speech therapists, dietitians, recreational directors and social workers.
Assisted-living residences are multi-unit facilities that provide assistance with medications and daily activities such as bathing and dressing. Additional amenities, such as meals, transportation and activities, are all included in a monthly fee.
